COMMUNITY REHABILITATION


 

BELGRAVIA CENTER

Belgravia Center is a transitional housing and reentry program that serves the greater Fresno County. The program provides housing, sustenance, life skills training, supervision, employment development training, and various other support services to male/female individuals referred by the agencies listed below for placement. Belgravia Center provides 24 hour supervision for the residential program.

Golden State BOP

Turning Point Residential Re-Entry program is located at 3545 S. Golden State Blvd. in Fresno, CA. The facility is co-ed and houses 40 inmates, while also providing supervision to 20+ residents on Home Confinement by way of Ankle Monitor GPS services. It is the mission of the Federal Bureau of Prisons to protect society by confining offenders in the controlled environments of prisons and community-based facilities that are safe, humane, cost-efficient, and appropriately secure, and that provide work and other self-improvement opportunities to assist offenders in becoming law-abiding citizens.

 

HOUSING SERVICES


 

GOLDEN STATE TRIAGE

This facility is a 50 bed, low-barrier housing option for those who are currently experiencing homelessness.

FAMILY VILLA

The Family Villa Program is a Permanent Supportive Housing program which houses 26 families. The program is funded by the U.S. Department of Housing and Urban Development and the County of Fresno. The program is designed to permanently house homeless families with at least one applicant experiencing a disability and homelessness (per HUD’s definition of disability and homelessness).

FALCON COURT

Falcon Court provides permanent supportive housing to 34 disabled chronically homeless individuals, and 6 chronically homeless families. The program offers case management, life skills training, education and employment assistance, linkages to community resources, and financial counseling.

BRIDGE POINT

Bridge Point is a 30-bed community-based transitional housing program for adults. It was established through cooperation and support from the following: The U.S. Housing and Urban Development Department, the City of Fresno, the State of California Housing and Community Development Department, the County of Fresno, Region I SASCA, and through private donations.

STASIS CENTER

The Stasis Center is considered a Permanent Supportive Housing Program and is funded by the U.S. Department of Housing and Urban Development (HUD). The apartments provide housing for individuals who were formerly homeless.

JOURNEY HOME

Journey Home Room Key Hotel is a 76 Room facility on North Parkway Drive. For low-barrier homeless individuals. Low barrier housing has no requirements concerning income, sobriety, or compliance with mental health treatment. Partners and pets are welcome and storage space is available for their personal belongings.

SAGE COMMONS

Sage Commons is a 105 room low barrier triage shelter for adults experiencing homelessness. Sage Commons staff utilize the housing first model, the belief that every person deserves housing regardless of barriers faced. There are no requirements regarding identifying documents, income, sobriety, nor compliance with mental health treatment. Shelter will be provided regardless of sexual orientation, marital status, or gender identification. Partners or adult families and their pets are welcome to room together.

THE WELCOME CENTER

The Welcome Center is a 30 bed dormitory style triage shelter serving individuals who are experiencing homelessness. The Welcome Center is a low barrier shelter defined as there are no specific requirements in regard to income, sobriety, or mental health treatment/compliance. Partners and pets are welcome and a storage area is set aside for a predetermined amount of personal belongings. The Welcome Center provides shelter, three daily meals, hygiene supplies, case management, and services that are housing focused.

 

ADULT MENTAL HEALTH


 

FIRST STREET CENTER (FSP)

The First Street Center-Full Service Partnership program is an MHSA funded Full Service Partnership serving approximately 105 adult individuals with severe mental illness (SMI), referred by the County of Fresno AB 109 Probation Department.  FSC-FSP provides comprehensive mental health/co-occurring services and works in conjunction with the Fresno County Probation Department to fulfill agreed upon treatment goals. FSC-FSP focuses on client strengths/abilities to successfully gain independence and self–sufficiency in the community.

RURAL MENTAL HEALTH

RMH services are funded by the Mental Health Services Act (MHSA) in partnership with Fresno County Department of Behavioral Health.

VISTA

Vista is a Full Service Partnership (FSP) program supporting clients in reaching their own vision of wellness and recovery. Vista provides outpatient based comprehensive rehabilitation and recovery services for adults (ages 18+).

SUNRISE

The Sunrise Programs mission is to provide comprehensive mental health care services to adults referred by Fresno County Department of Behavioral Health who live with mental health and cooccurring disorders. With a “Whatever It Takes” attitude, Sunrise staff strives to empower our consumers to achieve self-sufficiency and independence utilizing wellness and recovery and strength based principles. Our services are transitional in nature and include providing resources to our consumers by listening, motivating, advocating, teaching, and offering support in the most respectful way possible.

Diversion

The Turning Point Diversion Program is a “whatever it takes” and “meeting the individual where they are” program. The program is Assertive Community Treatment (ACT) and Full Service Partnership (FSP) model program supporting persons served in reaching their own vision of wellness and recovery. Services are delivered as an outpatient mental health program serving individuals diagnosed with a Serious Mental Illness (SMI), and at risk of being hospitalized, homeless, and/or incarcerated.


OUTPATIENT SUBSTANCE ABUSE

FIRST STREET CENTER-OP

The First Street Center-Outpatient program is an outpatient substance abuse program serving adult individuals in need of substance use disorder treatment and/or mild to moderate mental health support. Individuals are referred by the County of Fresno AB 109 Probation Department.  FSC-OP provides comprehensive mental health/co-occurring services and works in conjunction with the Fresno County Probation Department to fulfill agreed upon treatment goals. FSC-OP focuses on client strengths/abilities to successfully gain independence and self–sufficiency in the community.

FRESNO FEDERAL AFTERCARE

Turning Point offers aftercare substance abuse treatment and prevention and mental health treatment. The Aftercare program provides a comprehensive program of counseling and testing to facilitate the cessation of all illegal substance abuse for program clients. These drug abuse prevention, counseling, and mental health treatment services are provided under contract with the U.S. Court System.

QUEST HOUSE

Quest House is designed to decrease the return to custody rate and significantly lower prison overcrowding, and the ‘revolving door syndrome’. The primary purpose of the program is to help the ex-offender in becoming self-sufficient and productive. Parolees are provided the opportunity and encouragement to change and grow.
